Major Concepts Role as a Team Member with in the Discipline of Allied Health: This role involves a commitment to professional growth, continuous learning and self-development. This role includes high ethical and practice standards and understanding of the legal parameters of medical assisting practice. II. Sub-concepts Clinical Competence: Current knowledge of clinical concepts, procedures, and skills are necessary in the implementation of patient care. Clinical competence encompasses practice within the legal parameters as well as ethical responsibility deemed acceptable by the profession and by society. Clinical competence is based upon competencies in cognitive, psychomotor and affective domain using critical skills. Pharmacology: The study of pharmacology relates to an understanding of the general mechanisms of drug absorption, transport, physiologic effects, and metabolism and excretion. The medical assistant must also practice techniques of medication preparation and administration, which ensures accurate and safe delivery of ordered medication. Math skills are also required to calculate drug dosages. Human Needs: Human needs, according to Maslow, are arranged in a hierarchy with higher needs, such as self-actualization, emerging as lower needs are met. The knowledge of human needs then becomes a guideline in providing a safe, effective care environment. The relationship among different needs and variables that determine priorities of care are demonstrated in both theory and clinical application. Communication: Communication is an essential component in every step of the medical process. Information must be gathered, assimilated, and transmitted. Therapeutic communication must be mastered to facilitate assessment of both content and feeling and respond appropriately. The knowledge and application of communication theories and techniques assists the medical assistant in establishing a working relationship with individual patients and members of the health care team. Developmental Stage: Knowledge of growth and development assists the medical assistant in recognizing commonalities and variations in the developmental sequence. The medical assistant must then apply medical planning in the context of the individuals developmental stage. Societal Influences: Many variables diversity, sociocultural, demographic, religious, political, economic, ethical, and legal all comprise societal influences, which impact health care are discussed by students in the curriculum. The changes in health care delivery such as shortened hospitalizations, increased illness acuity; the increasing cost in health care, the growing population of elderly; and the increased life expectancy for individuals experiencing chronic health and social problems provide future challenges for medical assistants. III. Physical and Cognitive Expectations of a Medical Assistant Personal Characteristics: The ability to perform under pressure in stressful and emergency situations is a quality essential to CMAs. A stable temperament, strong sense of responsibility, considerable patience, and concern for order are required. Manual dexterity and physical stamina are vital. They must be able to work quickly, but accurately, and must be oriented to detail, yet able to integrate a number of activities according to priority. They must be keenly sensitive to the needs of the patient as well as to the needs of other members of the health care team in the medical office. Individuals who practice this profession have a strong desire to help others and make a valuable contribution to society. Working Conditions: CMAs work in clean, brightly lit, cool environments. At times they may be exposed to communicable diseases and unpleasant sights, odors, and materials. Most of their duties require standing, sometimes for a number of hours, and it is imperative that their attention be focused closely on the task at hand. Most procedures are short and the field of medical assisting requires a great turn over in patients as well as a fastaced environment. A 40-hour week is common, although some late hours and weekend time is necessary in case of emergency or for those who work in urgent care centers. ADMISSION Students are admitted to the medical assisting program each semester. The curriculum course sequence has been developed around students beginning with Fall Semester. Certain courses are only offered once a year. It is preferred that students complete developmental courses based upon their placement scores before entering MED-prefix courses in the curriculum. They are considered for admission only after all admission requirements have been completed. Admission criteria are as follows: A completed Application for Admission form. Evidence of high school graduation or completion of GED certificate. Official copies of high school and college transcripts. Satisfactory scores on the college placement test or satisfactory remediation of areas not meeting cutoff scores. Students must have completed a medical physical and history form including all required immunizations prior to enrolling in MED 150, and MED 260. This includes having a current TB skin test at all times. Malpractice/liability insurance is required for the program. The fee for this insurance is incorporated into the students tuition and fees for both of the years enrolled. Students are officially accepted into the Medical Assisting program at the beginning of the first semester their first year when they enroll and have successfully completed MED 110- Orientation to Medical Assisting An applicant may be denied continual enrollment in the MA program for any of the following reasons: Physical or emotional health that may indicate impairment in the ability to provide safe care to the public. Conduct not in accordance with the standards of a Medical Assistant: Has given false information or has withheld material information Has been convicted of or pleaded guilty or nolo contendere to any crime which indicates that the individual is unfit or incompetent to practice medical assisting or that the individual has deceived or defrauded the public Uses any drug to a degree that interferes with his or her fitness to practice medical assisting Engages in conduct which endangers the public health Positive Chest X-Ray that documents active tuberculosis c. Due to American Association of Medical Assistants (AAMA) requirements for certifying medical assistants, those applicants with felony convictions cannot sit for the national CMA exam, however there is an appeal process; in some situations. d. Violation of any policies in the 51 Student Handbook, according to stated policies. At the end of each Medical Assisting course for which a student is registered, he/she will receive a final grade. Students earn the following final grades using the following system in the Department of Health, Human & Business Services: LETTER GRADEDEFINITIONQUALITY POINTSA90-1004B80-893C70-792D60-691F<600Grading policies may vary from instructor to instructor. Policies are given in each course syllabus. Read your grading policy on each course syllabus. Below are general Medical Assisting grading policies, however ALWAYS follow each instructors given policies for a class. When calculating final grade point averages for each class, decimals, up to one place, will be carried until the final grade is calculated. When the final grade is calculated, the number will be rounded up to the nearest whole number if the tenths place is 0.5 or greater. For example, 87.5 will rounded up to 88, but tenths place less than 0.5 will not be rounded up, therefore an 87.4 would be 87. Percentages for tests, assignments, projects, etc. used to calculate final grade point average are listed on your course syllabus. All assignments/projects must be computer-generated. Hand-written assignments will not be accepted. Assignments will not be accepted after the designated due date and time. All assignments, homework and projects will have a due date announced in class, documented on the course outline or posted on Moodle. All assignments, homework and projects will also be graded on spelling and grammar. A one-point deduction will occur for misspelled words or incorrect grammar. Rationale: Medical Assistants must be timely in their work to provide quality care for their patients, and all documentation in the medical or financial record must be accurate and correct because they are legal records. Most facilities have converted from manual, written data to electronic medical records/billing systems. The student should develop excellent computer skills with attention to accuracy in their work. Test dates will be posted on the syllabus or announced in class by the instructor. If a student misses a test, the student will receive a grade of 0 on the test. Follow the syllabus and the schedule of assignments found within each course to identify dates and policies which include assignments and testing requirements When a given course has psychomotor and affective competencies stated in the course syllabus objectives, 100% of all Clinical psychomotor and affective competencies in the course must be successfully passed according to the competency criteria in order to pass the course, regardless of the final grade point average in the class. All of these competencies will be assessed according to a Pass/Fail basis as described in each competency criteria. A course must be repeated if 100% of course competencies in the psychomotor and affective domain are not passed. Cognitive objectives will be measured by course tests, assignments and projects. Each course requires a passing grade point average of a 2.0 or equivalent to a C at the end of the semester, or the course must be repeated. All graduates must complete 45 semester hours as stated on the student Medical Assisting progress sheet. All graduates are required to have a minimum overall 2.0 grade point average. All graduates must complete a graduation application. Medical Assisting students must make a grade of C or better in all MED-prefix and OST-prefix courses. A student who does not make a C or better is required to repeat the course. To be placed in MED 260 during the last semester, the student must have an overall grade point average of 2.0 and completed all required class and permission of MA program manager. MA research papers or projects must be typed, double-spaced, formatted in APA format with a bibliography containing references used or cited. Students must also log on to HYPERLINK "http://www.haywood.edu" www.haywood.edu to set up and access their campus email account. All communication with the student occurs through campus email ONLY. EVALUATION All faculty at Haywood Community College has academic freedom in their individual classroom, therefore policies may vary according to each instructors syllabus. Each student is responsible for following the syllabus and schedule of assignments for each class. Course competencies MAERB Competencies This course is part of the 51 Medical Assisting Program curriculum, which is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP), upon the recommendation of the Medical Assisting Education Review Board (MAERB). MAERB establishes cognitive objectives, psychomotor competencies and affective competencies which serve as the foundation for all Medical Assisting courses. Cognitive objectives are defined as things that a student is expected toknow. Examples may include definitions, terminology, concepts or steps to a process. These objectives are typically measured with worksheets, quizzes, tests and other written assignments. Psychomotor competencies are defined as things that a student is expected todoordemonstrate, such as providing first aide, assessing a blood pressure, or completing a bank deposit. These competencies are typically measured with skill check sheets and simulations. Affective competencies are defined as the way a student is expected tobehaveorrespondto a situation. Examples may include respect, confidentiality and sensitivity to patient needs. These competencies are typically measured with case studies, role play scenarios, online discussion forums, clinical evaluation tools, and simulations. MAERB objectives and competencies are noted in the schedule of assignments and are designated by a combination of letters and roman numerals (example - I.C.V). The letter indicates the type: cognitive objective (C), psychomotor competency (P), or affective competency (A). All cognitive objectives and psychomotor and affective competencies will be taught and assessed. All psychomotor and affective competencies must be passed with a minimum threshold score of 80% or higher, all clinical competencys must be 100%. The specific directions for each competency assignment will specify the minimum threshold score. Students will be given two (2) attempts to achieve a passing grade for each competency. Medical Assisting students who fail to meet the minimum threshold score for any competency within two (2) attempts will be referred to the Medical Assisting Program Manager. Failure to achieve a passing grade on a psychomotor or affective competency will result in a required remediation plan or failure of the course. Accreditation requires that Medical Assistants successfully complete all general, clinical and administrative competencies with a passing grade in cognitive, psychomotor and affective domains. These will be documented in each course syllabus. In order to successfully complete a Medical Assisting course, these outcomes must be mastered satisfactorily. Regardless of grade average in a course, the student will fail the course, if 100% of all competencies in the course are not successfully passed according to the criteria on the competency. (See appendix 2) Lecture At the discretion of the instructor and as stated on each individual course syllabus and outline, the instructor will give routine feedback to the student on progression in the lecture component based upon exams. On the average, there should be a 48-hour turn-around time on grades. The number of exams should be equal to or greater than the course credits. At the discretion of the instructor, the instructor may change the course syllabus and outline to meet the individual needs of a class. However, these changes should be announced in advance or posted on Moodle for the student. Cell phones are not allowed for use during class or testing. Lab Lab components will be combined with some lecture classes. Designated time will be set aside in these classes for demonstrating, performing and evaluating clinical procedures. Students are required to wear scrub uniforms, closetoed shoes and socks, and nametags to MED 150, and MED 260. Student will need to wear nametags, closetoed shoes and socks to MED 140. Students will also be evaluated on professionalism, cleanliness, use of Personal Protective Equipment (PPE) and organization during lab classes. Competencies will be evaluated in the lab components of the course. Students must take initiative to practice skills repeatedly until they have mastered the skill. Ample time in lab is set aside for practice. Attendance Attendance is mandatory for Medical Assisting due to the large amount of material covered and the extreme importance of learning information that is detrimental to the quality of care given to the patient. There are no excused absences. Each student should find a classmate in class for obtaining notes/lab content missed when absent. The student should call or email the instructor immediately when an absence is going to occur. Communication is an important part of professionalism. The student should not be late to class because the class will begin and end on time and disruptions affect others in the class. This will count as an absence. All lab classes will be scheduled on Tuesday nights from 5pm-9pm. Attendance will be recorded at the beginning of each class through a brief quiz, which will count toward your final grade. To receive credit, you must be seated and logged into your computer by 5:00 PM. If you arrive after the door has been closed, you will be required to wait in the lobby until the instructor opens the door. Late entry is at the discretion of the instructor and is not guaranteed. Rationale: Medical offices do not accept numerous tardies or absences by an employee and may end in termination of employment. Professionalism Each class will have a professionalism component. Each student should be prepared for class, motivated, compassionate and present and on time for class each day. Examples are cell phone going off, being disruptive in class, constantly talking in class particularly when the instructor is speaking, being absent, being late, not being prepared for class with assignments, being aggressive or disruptive to classmates or instructor, not putting away supplies after lab, not using required PPE, not cleaning work area, etc. If a student is aggressive or disruptive, the instructor may call security, and the student may be asked to leave the classroom. No cell phones will be allowed in class at any time. They need to be put away during class time. Occupational Risks faced by medical assistants include: a. Infectious agents: high risk of contact with infectious agents due to the various types of activities involved with their jobs b. Chemical Hazards: exposure to potentially hazardous chemicals used to clean and disinfect work surfaces, fixatives for specimens, drugs/treatments used for patients and latex. c. Physical hazards: musculoskeletal disorders due to manual lifting, transferring or repositioning patients and working in awkward positions. d. Work stress: created by emotional situations, exposure to human suffering, demanding physical work and risk of injuries, risk of exposure to infectious disease and hazardous chemicals. HYPERLINK "https://www.cdc.gov/niosh/healthcare/?CDC_AAref_Val=https://www.cdc.gov/niosh/topics/healthcare/default.html"https://www.cdc.gov/niosh/healthcare/?CDC_AAref_Val=https://www.cdc.gov/niosh/topics/healthcare/default.html CLINICAL PRACTICUM MED 260-MED Clinical Practicum is a 5-credit hour class where the students participate in a 240-hour clinical practicum in a medical office during their last semester. This is scheduled for Fall Semester of the second year, if the student stays on track with the curriculum. These days will be set by the job site and need to include at least 2 8-hour days a week. These days should be days that the job site can benefit the student with the most hands on learning. All MED-prefix courses except those taken during the same semester as co-requisites must be completed prior to enrolling in MED 260-Clinical Practicum and program managers permission. Criteria for satisfactory/unsatisfactory performance in the clinical practicum are based primarily on three factors: Consistency of performance Progressive independence of the student in medical duties Ability to perform in the medical environment competently and independently. If a student is discharged from the Clinical Practium, you will have one (1) more attempts to complete the clinical component the next time it is offered. Students must have a cumulative GPA of 2.0 to enroll in this course. As the student progresses through the program, he/she is expected to master competencies that are mandated by the standards of MAERB and CAAHEP. Students are graded by their clinical preceptor/physician at each clinical site by a college evaluation form. This grade is due upon the completion on the 240-hour clinical practicum. The Practicum Coordinator will visit each clinical site, make phone contact with the clinical preceptor at the site or email communication during the semester to evaluate the performance of the student. The student will wear scrub uniform, socks and shoes, and 51 name tag at all times while at the clinical site. Attendance is mandatory. The student is allowed two absences and should not be late. Rationale: All office managers in the area were surveyed about attendance. The majority stated that absences or tardiness by the student during clinical practicum were unacceptable. Employers look for motivation, positive attitude, and dependability as three traits in future employees. GENERAL INFORMATION Drug Screens and Criminal Background Checks Prior to enrolling in MED-260 Medical Assisting Clinical Practicum, students must submit to a criminal background check and urine drug screen test during the 60 day period before the first day of the clinical practicum. This is at the request of the clinical facility. The results will be reviewed by the clinical facility prior to accepting the student for the clinical practicum. If the clinical facility refuses to allow the student to participate in clinical experiences, for any reason, such as a positive criminal background check or drug screen, the student will not be able to progress in the program. This is done through Wolf One, Inc. . Some clinical sites may also require an on-line orientation before beginning the clinical practicum Criteria for Determining Unsatisfactory Clinical Performance at Externship After Initial Clinical Orientation, the student will be assessed as unsatisfactory in progression based upon the following criteria: Does not meet the clinical outcomes. Exhibits inability to perform designated psychomotor skills safely. Violates professional ethics such as, but not limited to: 1) carelessness with patient confidentiality, 2) violates agency policies frequently, 3) inability or unwillingness to recognize implication of behavior on practice. Insensitive to the needs of patients/office staff. Does not utilize appropriate communication skills with patients and/or members of the health care team. Requires an unreasonable length of time to plan, organize, and implement a procedure or requires repeated guidance in performance. Exceeds unacceptable number of absences or tardies. Does not display signs of professionalism such as lack of compassion for the profession, poor attendance, being late, lack of respect for office staff, instructors, preceptors and classmates, spreads rumors or idle gossip or does not wear the designated uniform. Use of drugs or alcohol while at the clinical site. Students who appear to be on drugs or have the smell of alcohol on their breath will be asked to leave the clinical site and requested to have a drug test or alcohol test. Refusal will result in automatic withdrawal from the program. Any student who breaks confidentiality or HIPAA regulations will automatically be removed and withdrawn from the program. Behavior The Medical Assisting student is expected to conduct himself/herself in a professional manner at all times. When addressing classmates, instructors, co-workers, patients, and visitors in the clinical area, or anywhere on premises of the site, appropriate title and surname are to be used unless office policy indicates otherwise. A display of ill temper on the part of the Medical Assisting student is inexcusable, even under trying conditions or situations. The student must remain in control of his/her emotions. The quality and tone of the voice should be quiet, pleasant, and assuring. Use of profane, obscene or inappropriate language or behavior will not be permitted at any time. A student who does not adhere to the behavior code will be asked to leave the clinical site/classroom and receive an unsatisfactory for the day or may be reviewed for dismissal. Repeated offenses at any time during the progression in this program will be cause to question whether the student is able to accept personal and professional responsibility and will result in review for dismissal. Student Illness/Injury At no time and under no circumstances will Haywood Community College or the clinical affiliation site be responsible for any illness that the student may incur during the program or scheduled clinical experience. Each student is covered by a professional liability/malpractice insurance policy through the college while enrolled in the program which will cover injuries but not illnesses obtained during the program that are in the classroom or clinical experience. Students must have medical insurance or cash to pay for any medical illnesses that occur during the program. Student should always follow established guidelines by OSHA to prevent injuries and illnesses. Responsibilities of Students with Infectious Diseases Students are required to refrain from direct patient contact, if they have any condition that is transmissible by contact or droplet spread. Students with infectious skin conditions of the hand should refrain from direct patient care. Students with infectious conditions having airborne droplet spread should wear mask or refrain from directly working with patients. Each student is required to have a current flu shot and other medical documentation discussed later. As part of their education and preparation for the clinical experience, students will be instructed in the use of standard precautions and other OSHA precautions applicable to individuals with infectious diseases. Proper use of Personal Protective Equipment (PPE) will be demonstrated and used when working the body fluids. These precautions will be reviewed with students throughout clinical courses. The specific infectious disease policies of the facility or agency providing the clinical experience will be reviewed with the student prior to assignment in that facility. The student must uphold OSHAs Bloodborne Pathogen Standards at all times. Health Requirements The student must be complete each of the following items and submit to the program manager by the end of summer semester.. Each of these items must be completed prior to enrolling in MED 150 and MED 260. A physical examination must be uploaded prior to the end of Fall semester to participate in Laboratory Procedures (MED 150) in the spring semester. Current PPD for tuberculosis, if a previous positive PPD, the TB surveillance questionnaire with a chest x-ray is required. Varicella, series of two immunizations or a positive titer Hepatitis B, series of three immunizations or a positive titer DTP, series of three immunizations with proof of a current Tetanus booster MMR, series of two immunizations Influenza immunization (required in fall semester prior to externship) All forms must be dated and signed by provider. The student must be assessed by the provider documenting that the students physical and emotional health are acceptable for activities of a health profession as stated on the back page of the medical form. A printout of immunization records from the state registry is accepted in lieu of completion of the immunization form. Standard Precautions Guidelines Personal hygiene must be followed at all times with special emphasis on hand washing technique. Gloves must be worn for any direct contact with any blood or body fluids. Students who have exudative lesions or weeping dermatitis should refrain from all direct patient contact and from handling patient-care equipment and devices used in performing invasive procedures until the condition resolves; a release from a physician will be necessary before the student can resume direct patient care duties. In the event a patient is exposed to a students blood or body fluids or vice versa, the student will immediately report the incident to clinical preceptor at the site and Medical Assisting Program Manager at the college. College incident report forms are filed with the program manager. A student is ethically obligated to undergo testing for bloodborne pathogens when a patient has been clearly exposed to the students blood or body fluids or vice versa. Students should also comply with current guidelines for disinfection and sterilization of reusable devices used in invasive procedures. Hepatitis B is more common in clinical areas than is HIV infection. Although individually less lethal than HIV, hepatitis cumulatively accounts for 200-300 deaths per year among health care workers. A vaccine can prevent Hepatitis B. It is required that students have completed the immunization series prior to clinical contact. Infectious Diseases Students and faculty will be governed by the infectious disease policy of the agency or facility with which they are currently associated for their clinical experience, as well as the policies of Haywood Community College. Faculty members will inform students of the agency policy and the course of action to be taken by students if an incident occurs. Cardiopulmonary Resuscitation (CPR) CPR certification must be completed PRIOR TO ENTERING the medical assisting clinical practicum. Students must maintain CPR certification through the completion of the clinical practicum. In the event the certification expires prior to this time, it is the students responsibility to pay for and complete recertification. The club is SAMA membership is open to current students enrolled in Medical Assisting at 51 and meets at least once a month. The date and time of meetings are TBD by the officers once established. Meetings should last at least 30 minutes. Officers are elected by members. The goals of the SAMA club raises money for community projects as well as help with educational trips and sponsor the annual pinning ceremony. The MA pinning ceremony is a student recognition of their successful completion of the Medical Assisting program. The student is publicly awarded a lapel pin which specifically represents their program of medical assisting. The specific arrangements for the pinning ceremony will be made by a committee established by the SAMA club, or the Program Manager in the case of SAMA club absents. Transportation Each student is responsible for his/her own transportation to and from the college and all clinical facilities utilized for learning experiences. Arrangements for transportation should be made prior to entering the program, as excused absences will not be granted for transportation problems involving poor planning by the student. Childcare Children are not allowed in 51 classrooms or at clinical sites. Students should have a childcare plan in place prior to entering the program. Absences related to childcare issues will not be considered as excused. Employment Students who are employed are encouraged to allow adequate time to complete school assignments according to deadlines set forth by each course. Absenteeism due to work will not be excused. HIPAA-Health Insurance Portability & Accountability Act The student is educated on confidentiality at all times throughout the program in all classes. The student must follow HIPAA guidelines with regards to confidentiality. Prior to beginning the clinical practicum, students will be required to sign the MA Clinical Practicum Confidentiality Agreement Form. See Appendix IV. Failure to do so may result in dismissal from the program or clinical practicum site. Social Media The student is expressly prohibited from taking any cell phone photos and/or videos while at the MA practicum site. The student is expressly prohibited from posting any photos, videos, mention, references or details regarding the practicum experience to social media sites. Social media sites include, but are not limited to: Facebook, Twitter, Instagram, or YouTube. Upon graduation, all graduates are recommended to take the national certification exam administered by the American Association of Medical Assistants (AAMA). The application may be accessed at the national website, HYPERLINK "http://www.aama-ntl.org" www.aama-ntl.org, and completed online with a credit card or the student may print a copy the application from the website and mail the form. The cost to the applicant is $125 which the student must pay out-of-pocket. Once the application is received by AAMA a testing date will be assigned in about 30 days and the AAMA will email the applicant a letter verifying receipt of application and an authorization test code to schedule the test at a testing center. It is a 180-multiple choice questions from general, administrative and clinical areas. The applicant has a 90-day window to set up the exam with the testing center and to take the test or forfeits the $125. If the applicant passes the exam, he/she becomes a Certified Medical Assistant, CMA (AAMA) which is required by most employers. The credential expires in 5 years from the date taken. The CMA can either accumulate 60 continuing education units (CEUs) in those 5 years to renew the CMA or must take the exam again. The CMA (AAMA) is nationally recognized and accepted in all 50 states as the gold standard in Medical Assisting. AAMA All students are encouraged to join the AAMA as a student member while enrolled in the program. The cost is $30 per year or $55 for two years. Student memberships may be acquired and then after graduation, the student becomes an active member. The membership dues pay for tri-level membership at the national, state and local levels. Students at 51 are encouraged to attend monthly meetings of the local chapter. The Haywood County chapter meets the third Tuesday night of each month at 51 at 6:30 p.m. in the Juniper Building room 6105. The website for national is HYPERLINK "http://www.aama-ntl.org" www.aama-ntl.org and the North Carolina State Society of Medical Assistants (NCSMA) is HYPERLINK "http://www.ncsma.org" www.ncsma.org. In addition, programs must demonstrate that students have passed those competencies by tracking their successful completion in order to illustrate programmatic summative measures in Standard IV.B.1. The type of tracking mechanism can vary according to the program. Some programs use a Master Competency Checklist, some outline all the competencies in a detailed gradebook, others have an electronic form, and there are variations within all of those methods. All of those forms are acceptable, as long as programs are able to share the tracking form with the site surveyors and send it in to the MAERB office if requested for progress reports. Below is a template of a form that can be used as a tracking mechanism.
